If the reverse side of the form has information on it, a copy of the reverse side is also required.
(5) Fees
Customers are responsible for actual costs required for the procedure (costs for issuing official documents, photocopies, etc.) and communication costs such as postage when sending documents to us. The Company will bear the communication costs when sending documents from the Company.
(6) Cases in which the Company is unable to respond to a request for disclosure, etc.
We will confirm the contents of any request for disclosure that we receive and notify the requester of the results at a later date. However, we will refuse to respond to the request in any of the following cases. In such cases, we will inform you of the reason why we are unable to respond.
When we are unable to confirm the identity of the person making the request.
